(2017), 6 of 8 patients with MDD were considered responders at 12-month follow-up after initiation of the bilateral DBS of slMFB and 4 patients reached remission; long-term results revealed a stable treatment effect up to 4 years of follow-up. The lateral habenula seems to harbor hyperactive neurons in cases of depression, which could result in restrained activity of the noradrenergic, dopaminergic, and serotoninergic circuits connected to it (Li et al., 2011; Winter et al., 2011). Sartorius et al. (2010) reported results of DBS of the lateral habenula in one patient with therapy-refractory MDD, who reached remission after 1 month; of interest, sudden malfunction of the IPG led to transient recurrence of symptoms, which disappeared again as soon as neurostimulator was repaired.

Previous studies have indicated that the LHb exhibits increased firing and metabolic activity during depressed states (Shumake et al., 2003; Li et al., 2011, 2013; Cui et al., 2018), and reduction of LHb activity by deep brain stimulation (Sartorius et al., 2010; Li et al., 2011, 2013), pharmacological inhibition (Winter et al., 2011; Yang et al., 2018) and electrolytic lesions of the habenula (Amat et al., 2001; Yang et al., 2008) produces antidepressant effects in animal models of depression and patients with depression. Collectively, these studies from rodents and humans strongly suggest that hyperactivity of the LHb is a crucial factor for depressive symptoms.
